New Patient Registration
Thank you for your interest in joining Inspirada Pediatrics! Please review the information we need (listed below) and send an email to Schedule@InspiradaPeds.com. If you have a newborn to register and schedule a visit, please call and we will register them over the phone.
We highly recommend you call your insurance provider to confirm that we are in-network, as we may not be contracted with certain policies or they may have us at a lower tier-level and therefore you may end up paying more out of pocket. To find their phone number, look on the front or back of your insurance card.
If you have an HMO plan, you will need to call your insurance company to update your child's PCP (Primary Care Provider) to one of the providers at Inspirada Pediatrics (Dr. Cynthia Chase MD, Dr. Charles Hutchings DO, or Allie MacAdams PAC).
If your child is covered by two insurance companies, you need to contact both insurances to ensure Coordination of Benefits (COB) is completed (a process that determines which health insurance plan is responsible for paying for a medical claim when a patient has more than one plan).
We will call you within 2-3 business day to confirm registration and schedule you for an appointment. Thanks!
In order to schedule a NEW PATIENT APPOINTMENT, we will need the following:

INSURANCE CARDS (PLEASE INCLUDE A PICTURE OF THE FRONT AND BACK FOR ALL CARDS). If a child is covered by two policies, please send pictures (front and back) of both insurance cards.

PARENT DRIVERS LICENSE (PLEASE INCLUDE A PICTURE OF THE FRONT OF THE CARD)

INSURANCE POLICY HOLDER'S NAME, DATE OF BIRTH, AND PHONE NUMBER

CHILD'S NAME, DATE OF BIRTH AND GENDER (Please include all children in the family if there are multiple kids).

WE WILL CONTACT YOU DIRECTLY ONCE YOUR ACCOUNT(S) HAVE BEEN CREATED TO SCHEDULE YOUR APPOINTMENT(S). This will take 2-3 business days.

INSURANCE POLICIES THAT ARE HMO’S AND ALL MEDICAID PLANS WILL NEED TO BE UPDATED TO REFLECT INSPIRADA PEDIATRICS AS THE PCP (PRIMARY CARE PROVIDER). PLEASE CONTACT YOUR INSURANCE TO SELECT US PRIOR TO YOUR FIRST VISIT.
Failure to complete this step will result in your appointment being rescheduled.
